This Health Data Notice (“Notice”) applies to Balanced Life Supplements (“Company,” “we,” “us,” or “our”), a Texas-based business, and governs the collection, use, disclosure, and protection of Consumer Health Data in accordance with applicable health data privacy laws in the State of Washington and the State of Nevada.
